As America’s Physician Groups promote the virtues of risk-based, coordinated care in our discussions of healthcare reform with the administration and Congress, Case Studies continue to wow state and federal policymakers and legislators, purchasers, payers, foundations and advocacy organizations. This vehicle for sharing best practices has also stimulated a good deal of innovative thinking among our own members.
Case Studies in Excellence describes how coordinated care groups have achieved outstanding results, with the data speaking for itself. The publication has featured advances in chronic and preventive care, focused complex care, chronic pain management, and prevention of unnecessary hospital utilization, readmissions, and emergency room use. Other topics include improving patient satisfaction and other patient care metrics, team senior care, relationship-based care model, variation reduction, off-hours access, end of life care, social determinants of health, health disparities, HIT applications, community transportation, office staff motivation, and community wellness.
