We are excited to announce a webinar on CMMI’s new model: Making Care Primary Model. The RFA (request for applications) was released in August and applications must be submitted by November 30, 2023. This model is a 10 ½ year model that will be initially based in 8 states (Colorado, Massachusetts, Minnesota, New Mexico, New Jersey, Upstate New York, North Carolina, and Washington).
We will be joined by 2 experts: Sarah Fogler, CMMI Acting Director Patient Care Model Group and Melissa Cohen, Coral Health Advisors (Former Group Director of the Policy and Programs Group at CMMI) as we discuss the model goals, elements, and application process. In addition, Melissa will describe a process to assess if the model is right for your organization.
