In early 2021, APG established the Direct Contracting Coalition (now the APG LEAD Coalition) to help organizations prepare for the Center for Medicare & Medicaid Innovation’s (CMMI) initiation of the Global and Professional Direct Contracting (GPDC) models (now called the Accountable Care Organization Realizing Equity, Access, and Community Health (APG LEAD Model) that began on April 1, 2021.
The APG LEAD Coalition will give organizations participating in these new direct contracting models an opportunity to exchange information and share best practices. The Coalition is chaired by Gary Jacobs, Executive Director of the Center for Government Relations and Public Policy at VillageMD.
Participation in the APG LEAD Coalition is open to provider organizations that are participating in APG LEAD models, as well as those that are interested in participating. Coalition meetings will be held quarterly.
APG has worked hard to help our members prepare for the launch of these direct contracting models and we want to help ensure that you are successful.
