APG is excited to announce the publication of Case Studies in Excellence 2025. As part of APG’s Annual Fall Conference 2025 in Maryland, we are hosting a “Case Studies and Cocktails” session, which offers conference attendees the opportunity to learn more about each Case Study by interacting with program staff from the author organizations. APG’s Annual Spring Conference 2026 in San Diego will include a Learning Fest on the main stage. Join us to celebrate the achievements of each of our Case Studies in Excellence programs and participating APG member organizations.

About APG Case Studies in Excellence

Since 2011, APG has published a periodic compendium of deep-dive case studies that demonstrate innovative, high-quality health care initiatives designed and implemented by APG member organizations. Each annual edition highlights the exceptional value-based care our members provide to their patients and aims to share best practices across the APG membership and with other organizations. For policymakers, purchases/payors, and other health care stakeholders, Case Studies in Excellence also helps tell the story of what “value” looks like in frontline clinics with patient-centered, high-quality care models.

Previous issues of Case Studies in Excellence have featured excellent examples of inpatient-ambulatory transitions coordination, preventive and chronic care innovations, end-of-life care navigation support, complex care coordination, among others.
